Contract Termination due to Moving Away from Germany Permanently

Good afternoon,
I'm moving away from Germany next week and I would like to terminate my contract with Telekom. I'm aware of the special kind of termination for this case but I'm not aware on how to proceed.
- Can I revolve this issue via the internet or do I must write a letter and annex the Abmeldung?
- If I do need to write a letter, what information is needed to be in this letter?
Can someone help me with this?

There's a form for that (because of course there is).
It's not going to be immediately though, cancellation when moving abroad is only possible with three months notice..
