Relocation request for internet as I am changing apartment

Hi, I am changing my apartment on April 30th from Troisdorf to Siegburg. I have to place a relocation request but I do not speak or read the German language. I have checked the relocation page by Telekom but its in German and very unclear. Please help in setting up my relocation of the internet to a new apartment. Thanks, Rupal Jain
