It's actually relatively easy to do this via the customer center.
There you have to log in, select the contract and then you can directly give notice.
In this community, users primarily help other users with questions and problems. We as "externals" have no access to customer data.
Nevertheless, I will call the team here, they can help if necessary.

Hi, I filled the form you linked yesterday. I selected a further date and choose the option to not to block my internet now. But my modem can not connect this morning. Could that be related? Or an unrelated and coincidental issue? Is there a problem right now with Internet service?

I got moved to Furstenberger str. in Jan 2023, but I still get emails stating my address as my previous one, Gormannstr. See the pictured attached.
Also how can I confirm my cancellation is progressing and how can I return the modem?

@Sven Ö. thank you Sven, i filled the form again and attached the Abmeldung form I filled to submit to Buergeramt. but everyday passes, extends my last day another day that I do not need. please call me anytime when convenient for you.
