Cancelling My Contract + A Possible Refund

Hello,
I am an American student who studied in Germany for six months. I had a monthly plan (2 GB) that I wish to end (I am now back home in the US). I thought the plan would end on its own (as I didn't have enough money on my account to renew the 10 euro monthly fee). At some point in the last few months., I must have accidentally selected an "automatic renewal", and I recently was notified that Telekom took 15 euros out of my credit card in order to renew the plan. I was not aware of this until a few days ago.
I can speak German, but it isn't my mother tongue and I find a lot of the "tech jargon" on this website to be confusing. Could someone instruct me on how to cancel my plan?
If possible, I also wish to refund the 15 euros that was charged to my card. Since I switched out my German/Telekom SIM card (and already have an American plan with Sprint) it is physically impossible for me to use my old Telekom plan. I have no idea when I will return to Germany/Europe.
@Karsten L. Could you possibly help me with this? I saw that you answered a similar question a while ago.

Sounds like it is about a prepaid contract MagentaMobil Prepaid M.
You can easily cancel the contract

Money already deducted from your credit card to the prepaid account cannot be transferred back.
But the current prepaid account you can get paid to a bacnk account (I guess only German bank account)

Hello Darius!
Sorry for the late reply. I see a cancellation form, but it requires me to also supply my bank info to transfer the remaining balance. I no longer have a German bank account (I closed it on February 28th before leaving Germany).
I take it there's no way to transfer the money to my American bank account. If not, is there a cancellation form that doesn't require my German bank info?

For help from @Darius W. it will be necessary that he can reach you via phone. Du assume that he will not be able to call a U.S. phone number.
